Stage 5 - Training Facility PDF Print
Assessment and training through a 12 week program to recognise the participants skills and abilities and then for them to be connected to a credited local business who would be involved in the championship scheme
 
Stage 1 - Referral PDF Print
A client will be referred to the programme via referrals or through other agencies. At this point, initial background data is catalogued from other relevant support agencies.
 
Stage 2 – Initial Assessment PDF Print
This stage is a brief over view assessment procedure to ascertain if the client requires further assistance and to what degree. At this point some clients are signposted to alternative service providers; others have simple problems which can be dealt with immediately without further referrals. Those who are interested in the “back to work” apprenticeship” move to stage 3.
 
Stage 3 - Diagnostic Assessment PDF Print
The diagnostic assessment involves a detailed recording of the client’s background including; perceived and actual needs, personal history archiving, and other circumstantial evidencing. It also includes the initial risk assessment which considers past history and likely risk and monitoring elements for their involvement in the training programme.

Stage 4 – Care Plan – Scaffolding and Mapping PDF Print
At this stage a draft plan is drawn up to indicate general care plan contents. This involves erecting the scaffolding that needs to be in place to direct the client to the best possible outcomes and a flow diagram generated which is person specific and timescales agreed with the client with initial targets set.
